Facility: Corewell Health Rehabilitation & Nursing Center - Kentridge
Contract: 13 Weeks (extensions common)
Schedule: 3x12s (6:30 AM - 7:00 PM) | 36 hours/week
Weekend Rotation: Every 3rd weekend

Position Overview
This role is for an LPN who is comfortable working in a long-term care environment and can balance efficiency with patient-centered care. You will be responsible for managing day-to-day nursing duties including medication administration, monitoring residents, and collaborating with the care team to ensure consistent, high-quality outcomes. The position offers a structured schedule with 12-hour day shifts and requires strong clinical judgment, organization, and the ability to adapt in a fast-paced setting.
Responsibilities

Provide direct patient care in LTC setting

Administer medications and treatments

Monitor and report changes in condition

Document care in EPIC

Collaborate with RN and interdisciplinary team
Requirements

Active LPN/LVN license
-
1+ year experience (LTC preferred)

BLS certification
-
PPD or Chest X-Ray (if applicable)

Ability to pass background and clinical screening
